Does insurance cover Zepbound?

Most commercial insurance plans cover Zepbound with prior authorization for adults with BMI ≥30 (or ≥27 + a weight-related comorbidity). Medicare Part D excludes anti-obesity drugs by federal law, so Medicare patients pay cash unless they qualify for Lilly Cares.

Commercial coverage rates have improved sharply in 2025-2026 as more employers added GLP-1 weight-management to their formularies, often with step therapy (must have tried 1-2 other anti-obesity drugs first).

Tier placement is usually specialty/tier 3 — typical copay $80-150/mo with the Lilly Zepbound Savings Card lowering it to $25.

Medicare Advantage and Medicare Part D currently exclude anti-obesity use. Tirzepatide for OSA (FDA-approved Dec 2024) is an emerging coverage path for some Medicare patients.
