Medicare's federal statute excludes weight-loss-only drugs from Part D — but T2D, cardiovascular risk reduction, and OSA indications are covered. Here is the practical 2026 reality plus the workarounds.
Ozempic (semaglutide) for type 2 diabetes — covered under Part D with PA. Mounjaro (tirzepatide) for type 2 diabetes — covered. Zepbound for OSA in adults with obesity — covered as of late 2024 with PA documenting AHI severity. Wegovy for MACE risk reduction in patients with established CVD + obesity — covered as of 2024 after the SELECT trial label expansion.
Wegovy or Zepbound for weight management without a qualifying medical indication. The federal Part D statute (42 U.S.C. §1395w-102) explicitly excludes drugs "used for cosmetic purposes or weight loss." Active legislative attempts to amend this have not yet succeeded.
Document a qualifying comorbidity (T2D, established CVD, OSA) — that pivots the medication to a covered indication. Some Medicare Advantage plans include supplemental wellness benefits; check your specific plan SBC. Cash-pay with manufacturer savings card is the last-resort path; $499/mo Wegovy is achievable for many.

Yes — as of late 2024, Zepbound is covered under Part D for moderate-to-severe OSA in adults with obesity. PA requires sleep-study documentation of AHI severity. This was the first weight-loss-class drug covered by Medicare.
