Common questions about GLP-1 medications

Which GLP-1 medication produces the most weight loss?
Tirzepatide 15 mg (Zepbound) leads the class with ~22.5% mean body-weight reduction at 72 weeks in the SURMOUNT-1 trial. Semaglutide 2.4 mg (Wegovy) averages ~14.9% at 68 weeks (STEP-1). Individual response varies — about 1 in 3 trial participants reached ≥20%; about 1 in 6 lost less than 5%.
How much does a GLP-1 cost without insurance in 2026?
Brand-name list prices run ~$800-1,350/mo. Manufacturer savings cards (NovoCare Wegovy, Zepbound Savings Card) cap commercial-insurance copays at $25-225/mo. Eli Lilly Direct sells Zepbound vials direct-to-patient at $349-499/mo cash. Compounded semaglutide from licensed 503A pharmacies starts ~$149/mo.
Is Ozempic FDA-approved for weight loss?
No. Ozempic is FDA-approved for type 2 diabetes management and cardiovascular risk reduction in adults with T2D. Wegovy is the same molecule (semaglutide) FDA-approved for chronic weight management. Off-label Ozempic prescribing for weight loss is common but technically off-label.
What happens if you stop taking a GLP-1?
Trial extension data (STEP-1 follow-up, SURMOUNT-4 withdrawal) show roughly two-thirds of lost weight is regained within 12 months of stopping. GLP-1 therapy for weight management is FDA-labeled for chronic use.
Is compounded semaglutide safe and legal in 2026?
Compounded semaglutide is legal when prescribed by a licensed clinician and dispensed by a licensed 503A pharmacy on a patient-specific basis. Mass identical-copy compounding ended when the FDA cleared the shortage in February 2025. Verify PCAB accreditation and US pharmacy licensure.
Which insurance plans cover GLP-1 for weight loss?
Most commercial carriers (Aetna, Anthem, Cigna, UnitedHealthcare, Humana, BCBS Federal) cover Wegovy and Zepbound with prior authorization documenting BMI ≥30 (or ≥27 + a weight-related comorbidity). Medicare Part D excludes anti-obesity drugs by federal law; diabetes use is broadly covered.
