The best GLP-1 for men: what the trial evidence actually supports

The honest verdict

For men, the decision often hinges on cardiovascular risk. Semaglutide (Wegovy) has the SELECT cardiovascular-outcomes evidence β€” a 20% MACE reduction in adults with heart disease. If maximum weight loss is the goal and CV risk is not the driver, tirzepatide (Zepbound) led on efficacy.

Drug selection is a prescriber decision. This table summarizes the trial evidence β€” it is not a prescription or a substitute for clinical judgment.

The cardiovascular angle is why "best for men" often means Wegovy

Men develop cardiovascular disease earlier and at higher rates than pre-menopausal women. That makes the SELECT trial especially relevant: it enrolled 17,604 adults with established cardiovascular disease and obesity/overweight (no diabetes), 72% of them male, and showed semaglutide 2.4 mg reduced major adverse cardiovascular events (MACE) by 20% over 3.3 years.

For a man with a prior heart attack, prior stroke, or peripheral artery disease plus a BMI β‰₯27, that cardiovascular evidence is a decisive reason to favor semaglutide (Wegovy) β€” and the FDA expanded the Wegovy label to this indication in March 2024. See our full SELECT deep-dive for the endpoint breakdown.

If cardiovascular risk is not the driver β€” a younger man without established heart disease seeking maximum weight loss β€” the efficiency argument favors tirzepatide instead.

Maximum weight loss: tirzepatide leads

When the single goal is the largest weight reduction, tirzepatide (Zepbound) has the strongest trial evidence: βˆ’20.9% mean loss at 72 weeks in SURMOUNT-1, versus βˆ’14.9% for semaglutide in STEP-1. The dual GIP + GLP-1 mechanism appears to drive the additional effect.

Sub-group analyses in both trials show men respond robustly, though the mean weight-loss percentage in men is often marginally lower than in women β€” largely because men carry more lean mass, which shifts the body-composition math. The absolute pounds lost are typically higher in men given higher starting weights.

Muscle preservation is the bigger lever for men

Men start with more skeletal muscle, and rapid weight loss threatens it. Up to ~30% of weight lost on any rapid-deficit protocol can be lean mass without deliberate protection. For men β€” who often care about strength and body composition, not just the scale β€” this is the single most important adjunct.

The protocol is the same regardless of drug: 0.7-1.0 g protein per lb of goal body weight daily, and resistance training at least twice per week from month 2 onward. This preserves the lean mass that sustains resting metabolic rate and reduces post-treatment regain.

Testosterone and GLP-1s: an indirect benefit

Obesity suppresses testosterone through several mechanisms (aromatization of testosterone to estrogen in fat tissue, disrupted hypothalamic signaling, and low SHBG). Weight loss reverses much of this β€” men with obesity-related low testosterone often see levels rise as they lose weight on a GLP-1, with the largest gains in those who lose the most visceral fat.

This is an effect of the weight loss, not a direct pharmacologic action of the drug. It is a reason men with obesity-related hypogonadism may see broader benefit, but GLP-1s are not a testosterone therapy and should not be framed as one. If low testosterone symptoms persist after meaningful weight loss, that warrants a separate endocrine workup.

Dosing and titration: the schedule for men

Both drugs escalate slowly to build GI tolerance. Wegovy: 0.25 mg weekly Γ— 4 weeks, then 0.5, 1.0, 1.7, and 2.4 mg (maintenance), each step ~4 weeks β€” reaching maintenance around week 16-20. Zepbound: 2.5 mg weekly Γ— 4 weeks, then 5, 7.5, 10, 12.5, up to 15 mg, no faster than every 4 weeks.

Men often carry more weight to lose and may benefit from reaching the higher maintenance doses (2.4 mg semaglutide, 10-15 mg tirzepatide), but the dose is titrated to response and tolerance, not body size alone. The starting doses are sub-therapeutic by design β€” the STEP-1 curve shows most loss accrues between weeks 20 and 44. A common male-pattern mistake is expecting fast early results and over-restricting food on top of the drug, which accelerates muscle loss.

Side effects and how to manage them

The common side effects β€” nausea, constipation, diarrhea, fatigue, sulfur burps β€” occur across both sexes; men report GI effects at modestly lower rates in trial sub-group data. Management is the same: slower titration, smaller meals, avoiding fried food and alcohol on injection day, and aggressive hydration.

Two things men under-report and should not: erectile or libido changes (usually the drug is neutral-to-positive here via testosterone recovery, but any concern is worth raising) and gallbladder symptoms (right-upper-abdominal pain after fatty meals). Gallstone risk rises with rapid weight loss regardless of the drug.

The muscle-loss risk deserves repeating for men specifically because it is the outcome most likely to matter to them and most preventable: protein 0.7-1.0 g/lb goal weight plus resistance training twice weekly preserves the lean mass that a poorly-supported rapid loss would strip.

What it costs

Brand-name list prices run near $1,000-1,600/month, but manufacturer cash programs cut that roughly in half: Wegovy $499/mo (NovoCare), Zepbound $349-499/mo (LillyDirect). Licensed compounded semaglutide via telehealth runs $99-249/mo. A covered insurance copay can be $0-25.

For men with established cardiovascular disease and BMI β‰₯27, the SELECT-based Wegovy cardiovascular indication is often an easier insurance approval than the pure obesity indication β€” a coverage advantage worth raising with your prescriber. When a GLP-1 is not the right choice

GLP-1s are contraindicated with a personal or family history of medullary thyroid carcinoma or MEN2. They are used cautiously with a history of pancreatitis, severe gastroparesis, or active gallbladder disease. Heavy alcohol use amplifies GI side effects and pancreatitis risk and is worth an honest conversation with your prescriber.

A GLP-1 also is not a substitute for treating the specific driver of your weight if there is one β€” untreated sleep apnea, certain medications, or a thyroid disorder can blunt response. An honest prescriber screens for these rather than just escalating the dose.

Frequently asked

Which GLP-1 is best for men with heart disease?

Semaglutide (Wegovy) has the strongest evidence for men with established cardiovascular disease. The SELECT trial (72% male) showed a 20% reduction in major cardiovascular events, and the FDA expanded the Wegovy label to this indication in March 2024. If you have prior heart attack, stroke, or peripheral artery disease plus BMI β‰₯27, this is a decisive factor.

Do GLP-1s affect erectile function or libido?

Usually neutral to positive. By reversing obesity-related low testosterone through weight loss, GLP-1s can improve libido and erectile function in men whose issues are weight-driven. The drug has no direct negative effect on sexual function in trial data. Any new or persistent concern is worth raising with your prescriber.

How much weight can a man expect to lose?

On the trial means, roughly 15% of body weight on semaglutide (STEP-1) and 20% on tirzepatide (SURMOUNT-1) at ~68-72 weeks. Men often lose more absolute pounds given higher starting weights, though the mean percentage can run marginally lower than women. Your result depends on adherence, protein, strength training, and dose reached β€” not luck.

Do men lose less weight than women on GLP-1s?

The mean percentage loss is often marginally lower in men in trial sub-group data, largely because men carry more lean mass. But absolute pounds lost are typically higher given higher starting weights. Individual variation dominates the sex difference.

Will a GLP-1 raise my testosterone?

Indirectly, yes, if your low testosterone is obesity-related. Weight loss reverses much of the testosterone suppression that obesity causes. This is an effect of the weight loss, not a direct drug action β€” GLP-1s are not testosterone therapy.

Should men worry about muscle loss on GLP-1s?

It is worth managing, not fearing. Up to ~30% of rapid weight loss can be lean mass without protection. Protein at 0.7-1.0 g per lb goal weight plus resistance training twice weekly preserves muscle. This matters more for men given higher baseline lean mass.
