Is Rybelsus (oral semaglutide) as effective as Ozempic (injectable)?

Rybelsus 14 mg daily is roughly equivalent to Ozempic 0.5 mg weekly for A1c reduction. For weight loss, injectable doses (up to 2.4 mg Wegovy) are substantially more effective than the current oral 14 mg max. Higher-dose oral semaglutide (25 mg, 50 mg) is in development.

Rybelsus uses a permeability enhancer (SNAC) to make semaglutide orally absorbable. Bioavailability is ~1%, so oral 14 mg β‰ˆ injectable 0.5 mg.

PIONEER trial program: A1c reduction with Rybelsus 14 mg was non-inferior to liraglutide 1.8 mg and similar to Ozempic 0.5 mg. Weight loss ~4-5 kg vs ~6 kg for the same comparison.

OASIS trial (oral semaglutide 50 mg for weight loss): mean βˆ’15% weight at 68 weeks β€” comparable to injectable Wegovy. Awaiting FDA submission/approval.
