GLP-1 receptor agonists with Sulfonylureas
Sulfonylureas force the pancreas to release insulin regardless of blood glucose. Adding a GLP-1 stacks two hypoglycemia mechanisms.
Hypoglycemia is the central concern — most prescribers reduce the sulfonylurea dose by half or stop it.

Per-GLP-1 breakdown
- Wegovysemaglutide 2.4 mgMajor
Mechanism: Sulfonylurea-induced insulin release is glucose-independent; GLP-1 effects stack on top, raising hypoglycemia frequency.
What to do: Most clinicians cut the sulfonylurea dose by 50% at GLP-1 initiation, then re-evaluate at 4-8 weeks. Many discontinue the sulfonylurea entirely once GLP-1 maintenance dose is reached.
