Month 1: the slow start (3-5 lbs)

You will lose 3-5 lbs in the first 4 weeks, which feels alarmingly slow compared to social media expectations. This is intentional — starting doses (semaglutide 0.25 mg, tirzepatide 2.5 mg) are subtherapeutic. They exist to build tolerance, not drive weight loss.

If you lose less than 2 lbs in month 1: usually fine, often water-weight artifact. If you lose more than 8 lbs: tell your prescriber — too-fast loss is the muscle-loss + ozempic-face risk window.

Month 3: acceleration (8-12% of starting weight)

By month 3 you are usually at the 1.0 mg (semaglutide) or 7.5 mg (tirzepatide) dose. Appetite suppression is meaningful. Many patients describe forgetting to eat. Loss accelerates to 2-3 lbs per week.

Month 6: peak loss (12-22% of starting weight)
