GLP-1 pen not working: manufacturer troubleshoot for Wegovy, Ozempic, Zepbound, Mounjaro

A stuck dial, a click without dose, cloudy solution, or a broken needle hub each has a specific fix. Here is the manufacturer-sourced troubleshoot, when to call the pharmacy for a replacement, and when the pen is fine and it is actually your technique.

Dial will not turn to the next dose

Most common cause: the pen has less remaining than the dose you dialed. Wegovy 2.4 mg pens contain exactly 4 doses; a 5th dose is not physically available. Same for Ozempic titration pens.

Do not force the dial past its stop. This can crack the internal mechanism, and Novo Nordisk / Lilly will not replace a pen with visible external damage.

Call your dispensing pharmacy. If the pen is under the manufacturer defect window (typically 48 hours from dispense) they will replace it directly. Otherwise the manufacturer patient-support line (Novo: 1-800-727-6500, Lilly: 1-800-545-5979) handles complaint reports and may replace.

Pen clicks but no drop appears at the needle

This is the "priming" issue. Every fresh pen needs a 2-unit air-shot with the needle pointing up, dial-and-inject until a droplet appears at the tip. If you skip this step on a brand-new pen, the first dose fires air, not drug.

Some patients only need to prime on the first dose of a new pen; check your pen's specific label β€” Wegovy fixed-dose pens auto-prime, titration pens do not.

If you primed and STILL get no drop, the cartridge may be defective or the needle is not fully threaded. Try a fresh needle first. If still no drop, replace the pen.

Solution looks cloudy, grainy, or off-color

Semaglutide, tirzepatide, and liraglutide should all be crystal-clear and colorless. Anything else β€” cloudy, milky, particulate, yellow-tinged β€” means degradation or contamination. Do not inject.

Most common cause: the pen was accidentally frozen. Semaglutide phase-changes irreversibly below 36Β°F. If your fridge is set colder than 46Β°F or the pen touched the freezer coil, discard.

Second most common: expired. Even in-fridge, semaglutide loses potency 4-6 weeks after in-use start. Check the discard-by date you wrote on the pen when you first primed it.

Needle will not attach or falls off

GLP-1 pens use standard pen-needle threading. Try a fresh needle first β€” bent needles are the most common cause.

If a fresh needle also fails, the pen's thread is cross-threaded or stripped. This usually happens when a previous needle was screwed on too fast at an angle. Replace the pen.

Never inject with a needle you had to force β€” you can shear the tip off inside your skin.

Injection is painful (cold-pen issue)

Cold semaglutide stings dramatically more than room-temperature semaglutide. Take the pen out of the fridge 30 minutes before injecting. Room temperature (68-77Β°F) is what the pain data uses.

Rotate injection sites every dose β€” the same spot week after week creates lipohypertrophy (a firm nodule) that hurts and slows absorption. Front of thigh, abdomen away from navel, and back of upper arm all work. Move β‰₯1 inch from the last site.

If injections still hurt after all of the above, the needle gauge may be too large. Ask your prescriber for 32-gauge 4mm needles β€” the shortest and thinnest available and appropriate for nearly everyone.

My Wegovy pen dial is stuck β€” is it broken?
Most often the pen is out of doses (Wegovy 2.4 mg pens hold exactly 4 doses). Do not force. Call your pharmacy for a replacement.
What if I injected air instead of Wegovy?
Not dangerous β€” the volume is tiny. But you missed the dose. Do not double-dose to make up for it; wait for the next scheduled dose and prime the pen properly next time.
Can I use a Wegovy pen past the fridge expiration date?
No. Efficacy drops sharply after the printed expiration. Off-label extension is not worth risking; your prescriber will authorize a replacement.
How do I know if my pen is defective?
Cloudy solution, dial that will not move at all from zero, needle that will not attach after 2 fresh needles, or a leak from the cartridge seal β€” any of these means defective. Call the manufacturer patient-support line to report + arrange replacement.
