Switching from Ozempic to Wegovy: a step-by-step guide

Both are semaglutide, but the doses, indications, and insurance coverage differ. Here is how to make the switch without losing weeks of progress.

They are the same molecule

Both Ozempic and Wegovy are semaglutide manufactured by Novo Nordisk. The only differences are FDA indication, pen color, and max dose. Pharmacologically there is no difference at equivalent doses.

That's why switching is mechanically simple — but the access pathway and side-effect profile at higher doses are not.

Why people switch

Most common reason: plateau at Ozempic max dose (2.0 mg). Wegovy's 2.4 mg dose adds another small lift in weight loss.

Second reason: insurance. Some plans cover only the indication-matched drug (Ozempic for T2D, Wegovy for obesity). Switching means matching pharmacy to indication.

The switching protocol

Step 1: New prescription from your provider — Wegovy 1.7 mg if you were at Ozempic 2.0 mg (matches the next escalation), or Wegovy 2.4 mg if your provider agrees you tolerate well.

Step 2: No washout. Inject the Wegovy on the day your Ozempic dose would have been due — same weekly cadence.

Step 3: Monitor for 2 weeks — Wegovy at 2.4 mg has slightly stronger appetite suppression and can re-trigger mild nausea.

Insurance and access

Wegovy coverage typically requires: BMI ≥ 30, or BMI ≥ 27 with a comorbidity (hypertension, sleep apnea, dyslipidemia). Many plans also require failed lifestyle intervention documentation.

If denied: Novo Savings card brings cash price from ~$1,350/month to $499/month for commercially insured patients (check eligibility quarterly).

Will I lose more weight on Wegovy than Ozempic?
At the same dose: no — same molecule, same effect. At the max dose: yes, marginally, because Wegovy goes to 2.4 mg vs. Ozempic's 2.0 mg.
Do I need to taper off Ozempic before starting Wegovy?
No. They are the same drug. Switch at the next injection day at an equivalent or one-step-up dose.
Can my pharmacy substitute Ozempic for Wegovy?
No — they are FDA-labeled separately and pharmacies cannot substitute one for the other. Your prescriber must write a new prescription.
What if my insurance won't cover Wegovy?
Common workaround: the Novo savings card brings the price to $499/month if commercially insured. Compound semaglutide was a workaround for many in 2023-2024 but is no longer FDA-permitted as of late 2024.
