Foundayo's list price at launch is approximately $695/month for a 30-day supply at the maintenance 36 mg dose. List price is what an uninsured patient sees at the retail pharmacy counter before discounts.

Lilly positioned this price aggressively. Comparable branded injectables (Wegovy ~$1,350, Zepbound ~$1,060) cost meaningfully more. The pricing reflects two factors: (1) orforglipron is a small molecule, cheaper to manufacture than peptide injectables, and (2) Lilly wants to capture market share in the oral GLP-1 segment before the inevitable generic semaglutide arrives in 2028-2029.

LillyDirect: $349-499/month for self-pay

Lilly's direct-to-patient pharmacy (LillyDirect.com) offers Foundayo at $349-499/month for self-pay patients. Pricing tiers by quantity:

LillyDirect requires a valid US prescription. You can order through their site or have a telehealth provider (such as Ro or Hims) coordinate with LillyDirect for fulfillment.

Commercial insurance: $25-100 copay

Most large commercial plans (Aetna, Cigna, BCBS variants, UnitedHealthcare) have added Foundayo to their formularies through 2026. Coverage typically requires:

  • BMI β‰₯30 (obesity), OR BMI β‰₯27 with at least one weight-related comorbidity
  • Prior authorization documentation submitted by your prescriber
  • Some plans require documented prior lifestyle intervention (3-6 months of diet/exercise)

Medicare GLP-1 Bridge: $50/month from July 1

The Medicare GLP-1 Bridge program (effective July 1, 2026) covers Foundayo at $50/month copay for eligible Medicare Part D beneficiaries. Eligibility: BMI β‰₯30, or BMI β‰₯27 with a qualifying comorbidity.
