How to inject Ozempic

Ozempic is a weekly subcutaneous injection. The whole process takes under 5 minutes once you have the rhythm. Below: the FDA-recommended steps from pen prep through disposal.

    Remove the pen from the fridge 15 minutes early

    Cold Ozempic stings on injection. Let the pen warm to room temperature for 15–30 minutes before use. Do not microwave or run under hot water.

    Inspect the medication

    Look through the window. The liquid should be clear and colorless. If it is cloudy, discolored, or contains particles, do not use it — call the pharmacy for a replacement.

    Attach a new needle (if your pen requires one)

    Some Ozempic pens use a fresh needle for each dose; others are pre-filled disposable. Follow the package insert. Always use a fresh needle to avoid contamination.

    Dial the correct dose

    Confirm the dose marker matches your prescribed dose. If the dial does not turn freely, the pen may be empty or jammed — do not force it. Call the manufacturer support line listed below.

    Choose an injection site

    Three approved sites: front of thighs, lower abdomen (avoid 2 inches around the navel), or upper outer arm (a partner can help). Rotate sites week to week — same spot every week increases the chance of lipohypertrophy (lumpy tissue).

    Clean the skin

    Wipe the injection site with an alcohol swab and let it air-dry for 5–10 seconds. Inserting through wet alcohol stings more.

    Pinch the skin and insert at 90°

    Pinch a small fold of skin gently between thumb and forefinger. Press the needle straight in at a 90-degree angle. Push the plunger / button fully — most pens click or chime when the dose is delivered.

    Hold for 5–10 seconds, then remove

    Keep the needle in place for 5–10 seconds after the dose finishes to prevent leakage. Remove straight out, then release the skin pinch.

    Dispose of the needle safely

    Cap the needle (if it has a removable cap) and drop it into an FDA-cleared sharps container. Never reuse needles. When the container is full, dispose per your state's sharps regulations — most states accept return-by-mail programs.

    Log the injection

    Note the date, dose, and site in a tracker or notebook so you remember which site to rotate to next week and confirm you actually took the dose. Many patients also photograph the pen to prove the dose was administered.

What if I forget my injection day?

Take the missed dose as soon as you remember, as long as it is within 5 days of the scheduled day. After 5 days, skip it and take the next dose on your regular day. Never double-dose.

Can I inject in the same spot every week?

No — rotate sites. Same-spot injection over months causes lipohypertrophy (lumpy fat tissue) which can affect absorption. Aim for at least 2 inches between consecutive injection sites.

Does it hurt?

Most patients describe it as a pinch or minor sting. Cold pens hurt more — warming the pen for 15–30 minutes and letting the alcohol fully dry both reduce discomfort. The needles are very thin (32 gauge).
