Why diarrhea happens on Wegovy

GLP-1 medications change gut motility unpredictably — for some people the result is faster small-bowel transit (diarrhea), for others slower colonic transit (constipation). Many people swing between the two during titration.

Timeline

Onset within the first 1–2 weeks of starting or escalating; most cases resolve within 4 weeks at a stable dose.

How to manage it at home

  • Hydrate aggressively — water plus electrolyte drinks to replace lost sodium and potassium.
  • BRAT diet (bananas, rice, applesauce, toast) for 24–48 hours if symptoms flare.
  • Avoid dairy, caffeine, alcohol, and high-fat foods during a flare.
  • OTC loperamide (Imodium) is generally safe; use lowest effective dose.
  • Probiotic foods (yogurt, kefir) can help once acute diarrhea settles.

Call your prescriber if:

  • Diarrhea more than 6 times in 24 hours or lasting beyond 48 hours.
  • Blood in stool or black/tarry stools.
  • Signs of dehydration: dizziness, low urine output, racing pulse.
  • Fever above 101°F alongside diarrhea.

Can I take Imodium with Wegovy / Ozempic?

Yes — loperamide (Imodium) has no known interaction with GLP-1 medications. Use the lowest effective dose for acute episodes; avoid daily long-term use without prescriber input.

Why does my GLP-1 cause diarrhea sometimes and constipation other times?

GLP-1 drugs slow gastric emptying and affect gut motility unpredictably. Different segments of the GI tract respond differently — your stomach and small intestine may slow (constipation) while your colon speeds up (diarrhea), or vice versa. This usually stabilizes by month 2–3.
