Eli Lilly launched Foundayo (orforglipron 36 mg daily tablet) for chronic weight management in adults on May 5, 2026. Foundayo is the first oral non-peptide small-molecule GLP-1 receptor agonist — distinct from Rybelsus (oral semaglutide, a peptide with strict empty-stomach + 30-minute fast rules).

Phase-3 ATTAIN-1 enrolled 3,089 adults with BMI ≥30 (or ≥27 + comorbidity) randomized to orforglipron 12, 24, or 36 mg vs placebo for 72 weeks. Mean weight reduction was 9.6%, 11.4%, and 12.4% at the three doses vs 1.0% placebo. GI side-effect profile was similar to other GLP-1 agents.

Key practical advantages: no empty-stomach requirement (vs Rybelsus); no injection (vs Wegovy/Zepbound). Launch cash price is $649/mo with a Lilly Direct savings card lowering to $499/mo for the first 90 days. Compounded versions are not legally available (orforglipron is patent-protected through 2038).
