What the spread reveals
The first finding: the cash market has fully fragmented in 2026. Three years ago the choice was brand-name pen at list price (~$1,300/mo) or nothing. Today there are at least four distinct supply chains, each with its own regulatory status, quality assurance posture, and target patient profile.
The second finding: compounded semaglutide from licensed 503A pharmacies has settled around $199-297/mo as competition intensified through 2025-2026. Below $199/mo, walk away β the provider either uses unverified API or operates outside US pharmacy licensure.
The third finding: Eli Lilly's direct-to-patient Zepbound vial program (launched mid-2024) is the most disruptive single move in the brand-name cash market. $349/mo for the 2.5 mg starter vial is below every brand-name telehealth provider listed here.
The fourth finding: pharmacy coupon platforms (GoodRx, SingleCare, CareCard) discount the brand-name retail price by 10-25%, but they don't approach the compounded or Lilly Direct floor. Cost Plus Drugs sells brand-name Ozempic at $287/mo when in stock β closest competitor to the manufacturer-direct floor.
Methodology
Prices captured 2026-05-25 through 2026-05-31 from each provider's public rate card or intake screen. Compounded providers' prices fluctuate with API supply and were snapshot at the median of the past 30 days. Manufacturer-direct prices are FDA-approved label cash prices. Pharmacy coupon prices are the median for ZIP 10001 (New York City) on the platform's lookup tool.
