The FDA-approved BMI threshold for GLP-1 weight-loss medications is 30 or higher — or 27 or higher with a weight-related comorbidity such as type 2 diabetes, hypertension, sleep apnea, or dyslipidemia. Wegovy and Zepbound are FDA-approved at BMI ≥27 with at least one weight-related comorbid condition. Without comorbidities, you would need BMI ≥30.

BMI is a screening tool, not a diagnosis. It does not account for muscle mass, bone density, or body composition. Talk to a licensed clinician about whether GLP-1 medications are appropriate for you.

GLP-1 BMI thresholds

The FDA approves Wegovy (semaglutide) and Zepbound (tirzepatide) for chronic weight management in adults with:

  • · BMI ≥ 30 (obesity), OR
  • · BMI ≥ 27 (overweight) with at least one weight-related comorbid condition such as type 2 diabetes, hypertension, or dyslipidemia

Insurance coverage for these medications often requires documentation of a failed lifestyle-intervention attempt and may apply stricter BMI thresholds than the FDA label.
