Who qualifies for Wegovy on the NHS?

NICE TA875 (March 2023) restricts NHS-funded Wegovy to adults with BMI ≥35 plus at least one weight-related comorbidity (T2D, hypertension, dyslipidaemia, obstructive sleep apnoea, or established cardiovascular disease). For people of South Asian, Chinese, Black African, African-Caribbean, or other minority ethnic background, the BMI threshold is lowered by 2.5 (to BMI ≥32.5). Treatment must be delivered through a Tier 3 specialist weight management service and is time-limited to 2 years.

Why is Mounjaro covered on the NHS for both diabetes and weight?

NHS England approved tirzepatide (Mounjaro) for T2D in 2024 and for weight management in March 2025 (NICE TA1026). The weight-management indication has the same BMI criteria as Wegovy (≥35 + comorbidity, or ≥32.5 for minority ethnic adults). Unlike the US, where Zepbound and Mounjaro are separately branded for weight and diabetes respectively, UK Mounjaro is the same product for both — pricing is set once per the NICE process.

Can I get GLP-1 medication privately without going through the NHS?

Yes. UK private prescriptions require a GPhC-registered pharmacy and a UK-licensed prescriber. MHRA criteria for private weight-loss prescriptions are typically BMI ≥30 (or ≥27 with comorbidity). No. MHRA regulates compounded medicines under Special Manufacturer Licences, and these are restricted to medical necessity (i.e., a patient cannot tolerate the commercial formulation). Compounded semaglutide for cost reasons — common in the US — is not permitted in the UK. Suspected unlicensed semaglutide products sold online are illegal.
