Does GLP-1 improve cholesterol?

AReviewed by Jane Novak, MD, MPH· Updated 2026-05-31

Yes, modestly. GLP-1 medications reduce triglycerides 10-20%, LDL 5-10%, and ApoB ~7%. Most of the effect comes from weight loss, but a small drug-specific effect appears to be present in subgroup analyses.

STEP-1 lipid panel changes at 68 weeks: triglycerides −22%, LDL −5%, HDL +5% (drug vs placebo).

SURPASS-2 and SURMOUNT-1 showed similar lipid improvements with tirzepatide, sometimes slightly larger.

Patients on high-intensity statin therapy typically continue it through GLP-1 weight loss; deprescribing decisions should wait until weight has stabilized for 3-6 months.
