Can I take GLP-1s if I have a history of medullary thyroid cancer?

Short answer

No. GLP-1 medications carry a black-box warning against use in patients with personal or family history of medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2).

All FDA-approved GLP-1 receptor agonists for weight management — semaglutide (Wegovy), tirzepatide (Zepbound) — carry a boxed warning derived from rodent studies showing thyroid C-cell tumors. The warning explicitly contraindicates use in patients with a personal or family history of MTC or MEN 2.

While the rodent finding has not translated to confirmed human risk, the FDA maintains the contraindication. If you have a personal or first-degree-relative history of either condition, discuss alternative options (orlistat, naltrexone-bupropion, bariatric surgery referral) with your clinician.
