GLP-1 receptor agonists with Opioid analgesics
Opioids and GLP-1s both delay gastric emptying. Combined use significantly increases constipation, nausea, and risk of small-bowel obstruction.
Aggressive bowel regimen is required. Consider non-opioid alternatives where clinically possible.
Also known as: oxycodone, hydrocodone, morphine, tramadol
Per-GLP-1 breakdown
- Wegovysemaglutide 2.4 mgMajor
Mechanism: Additive delay of gastric and small-bowel transit. Risk of severe constipation, ileus, and (rarely) small-bowel obstruction.
What to do: Prophylactic bowel regimen (osmotic laxative + stimulant) from day one of GLP-1 if opioids are ongoing. Reconsider opioid taper or non-opioid pain control. Stop the GLP-1 immediately and seek care for abdominal pain that lasts beyond 24 hours.
