How to switch between GLP-1 medications safely

  1. 1Talk to your prescriber first. Never switch GLP-1s without medical input. They will evaluate your current dose, tolerance, and efficacy and confirm the equivalent dose of the new drug.
  2. 2Use the chart above to find the equivalent dose. The conversions come from comparative trial data (SURPASS-2, SURMOUNT-1, STEP-1). Confirm with your prescriber before filling the new prescription.
  3. 3Wait 7 days after your last dose. For weekly injectables, this aligns to your normal injection day. For daily oral, take the last dose, then wait a full week before starting the new drug.
  4. 4Monitor the first 2 weeks closely. New or worsened GI side effects (nausea, abdominal pain) should prompt a check-in. Your prescriber may step the dose down and re-titrate.

Is there a 1:1 conversion between semaglutide and tirzepatide?
No exact 1:1 — different molecules. The clinically-equivalent doses are derived from comparative trial data (SURPASS, SURMOUNT, STEP series). Use the chart above for the closest match and confirm with your prescriber.
How long should I wait between stopping one GLP-1 and starting another?
Standard guidance: wait at least 7 days after your last weekly injection before starting a different GLP-1. This allows the previous drug to clear and reduces stacking-related GI side effects.
Do I need to titrate again when switching drugs?
Not always. If you were tolerating a high dose of the previous drug, your prescriber may start you at a clinically-equivalent dose of the new one without re-titrating from the bottom. If you had significant side effects, restarting at the lowest dose is safer.
Can I switch from oral semaglutide (Rybelsus) to injectable Wegovy?
Yes. Wait 7 days after your last Rybelsus tablet, then start Wegovy at the dose your prescriber matches to your prior efficacy/tolerability. Rybelsus 14 mg has rough equivalence to Wegovy 1.0 mg injection.
