Evidence-graded answer
Can I take GLP-1 after bariatric surgery?
Yes, in many cases. Post-bariatric weight regain affects 20-30% of patients within 2-5 years. GLP-1 added to bariatric surgery (sleeve, RYGB) has shown additional weight loss in observational studies and small RCTs. Discuss with your bariatric surgeon.
BARI-OPTIMISE pilot RCT (semaglutide 2.4 mg added to post-sleeve patients with insufficient weight loss): additional 9% body weight reduction at 52 weeks.
Mechanism overlap: bariatric surgery produces durable changes in endogenous GLP-1 and PYY; exogenous GLP-1 augments the effect rather than duplicates it.
Practical caveats: post-bariatric GI anatomy may amplify GLP-1 nausea early. Start at lowest dose, titrate slowly, and coordinate with the surgical team for any new GI symptoms.
