Evidence-graded answer

Can I take GLP-1 with warfarin or other blood thinners?

Yes, with monitoring. GLP-1 does not directly interact with warfarin, but rapid weight loss + changing food intake patterns can shift INR. Plan for more frequent INR checks (weekly during titration, then settle to monthly).

No PK/PD interaction between GLP-1 and warfarin, DOACs (apixaban, rivaroxaban), or antiplatelets.

INR instability comes from changing vitamin K intake (different food choices) and changing weight (warfarin dose is partly weight-based).

For DOACs, monitor for signs of bleeding but no routine lab adjustment needed. For warfarin: weekly INR for first 8 weeks, then every 2 weeks for 8 weeks, then back to monthly once weight stabilizes.
