Does GLP-1 work for type 1 diabetes?

No GLP-1 is FDA-approved for type 1 diabetes. Some endocrinologists prescribe semaglutide off-label as an insulin adjunct in T1D with obesity or insulin resistance, but evidence is limited and DKA risk during gastric-emptying changes requires careful monitoring.

T1D is autoimmune destruction of pancreatic beta cells; the body produces no endogenous insulin. GLP-1's main mechanism (glucose-dependent insulin secretion) cannot work in T1D.

Secondary effects (delayed gastric emptying, reduced glucagon, reduced appetite) can still help T1D patients with insulin resistance or obesity. Small trials show modest A1c reduction and weight loss as an insulin adjunct.

Use requires close glucose monitoring (CGM strongly recommended), insulin dose adjustment by an endocrinologist, and patient education on DKA warning signs.
