Is GLP-1 safe with kidney disease?
Yes for most patients with CKD stages 1-3. Semaglutide has FLOW trial data showing kidney protection. Doses do not require renal adjustment. Severe CKD (eGFR <15) is not a contraindication but warrants nephrology coordination, particularly during early titration when dehydration risk from nausea/vomiting is highest.
FLOW trial (NEJM 2024): semaglutide reduced composite kidney endpoint by 24% in 3,533 adults with type 2 diabetes + CKD. Strong evidence for kidney protection in this population.
No dose adjustment is required for any GLP-1 across eGFR ranges studied in trials.
Acute kidney injury reports in surveillance data are almost always linked to volume depletion from severe vomiting/diarrhea during titration. Mitigation: slow titration, fluid intake, anti-emetic ondansetron PRN.
