Cross-ingredient switch — re-titration required
Mounjaro uses tirzepatide; Wegovy uses semaglutide. You restart from the starter dose and re-titrate over 12–20 weeks to reduce GI side effects.
These medications use different active ingredients (semaglutide vs. tirzepatide). Re-titration from the starter dose is required to reduce GI side effects.
Stop Mounjaro. Start Wegovy 0.25 mg for 4 weeks, then escalate through standard 16-week titration to 2.4 mg.
GI symptoms during titration. Some patients see modest weight regain because Wegovy 2.4 mg generally produces less weight loss than Mounjaro 10-15 mg.
Wegovy PA requires BMI ≥30 (or ≥27 with comorbidity). Often easier path than appealing a Mounjaro denial.

Some weight regain is possible, especially if you were stabilized at high Mounjaro doses (10-15 mg). Wegovy maxes at 2.4 mg, which is generally less potent for weight loss than maintenance Mounjaro/Zepbound. Monitor weight monthly during the first 6 months.
